Deadline Extended: Supported Medical Decision Making Survey



Deadline extended to March 31, 2022.


We are looking for volunteers to take part in an online survey to evaluate a health planning document for people with intellectual disabilities created by Inclusion Saskatchewan. If you are a Self-Advocate, family member of someone with an intellectual disability, health professional, or a disability support professional, we ask that you:

Inclusion Saskatchewan worked with a group of Self-Advocates to develop the My Health: How to Support My Decision Making document, a new plain language resource that guides individuals and their supporters through the process of making supported medical decisions.


Feedback from this survey will be used to update and inform revisions of the resource, with a goal of more effectively supporting individuals with intellectual disabilities to communicate their preferences and needs when accessing health care services. A research committee, headed by Dr. Elise Matthews, PhD, RN, an Associate Professor with the Faculty of Nursing at the University of Regina, will be evaluating the feedback and use of this document.


Participation will take approximately 20 minutes.
